Private Lessons

Use this form if:

1) You are already a student at CCSM, planning to continue private lessons with your current instructor.

2) You have arranged a weekly lesson time and start date with your instructor.

* If you have not yet confirmed a weekly lesson time and start date with your instructor, please do so prior to completing this form. Current students requesting lessons on a NEW INSTRUMENT, please fill out this form.

Please note that a $20.00 deposit (per student entry) will be collected upon submission of this form.

This will be added to the monthly tuition statement for the family account. All lessons/classes must be paid for in advance.
