Description 45-Minute Private Lessons (Minimum of 12 Lessons) Please call the school office, if registering for less than 12 lessons. Requirements Student must have class day and time prearranged with instructor before registering. Hours & Sessions Arrange with instructor Lesson Start Date (For Private Lessons: Must have scheduled date and time prearranged with your instructor.)

Payment Options I will pay tuition, material and registration fees in full today. I will pay for the first 4 lessons, material and registration fees today. I authorize CCSM to charge future monthly payments to my credit card on the 1st of every month until balance is paid in full. I will pay for the first 4 lessons, material and registration fees today. CCSM will mail monthly statements. I will submit payment by check or credit card by the 1st of the month. ( A late fee of $25 will be applied if the payment is not received by the 10th of the month)
