Programs


Payment of Fees

The registration fee, applicable material fees and fi rst tuition payment are due at registration. Subsequent tuition payments are due on the 1st of each month, payments are considered late if not received by the 15th of the month. A late fee of $25 is automatically applied to each student account with a past due balance. A $25 service charge will be applied for checks returned by the bank for any reason.

Cancelled Classes

CCSM reserves the right to cancel group classes if enrollment does not meet the required minimum number of students. In the event a class is cancelled, the parent/student will be contacted and no payment will be processed (your check will not be deposited, or credit card charged until the class minimum is met).

Refund Policy

CCSM requires a written request for a refund to be considered. The $20 registration fee is non-refundable. There are no refunds or credits for “student missed lessons.” In the event of prolonged absence due to family illness or an emergency, refunds will be considered on a case by case basis.

Missed Lessons

Out of courtesy, students should notify the teacher of any absence for any reason. However, this does not exempt the student from payment. There will be no make-up lessons/classes for student absences. If a teacher is absent, a makeup lesson will be scheduled at a mutually convenient time. If the student misses a scheduled make-up opportunity, there will be no make-up lesson. However, payment will be required as in a regular student absence. CCSM is committed to pay the teacher for the time you have scheduled. To retain quality instructors, CCSM must pay the teacher whether the student attends the lesson or not. Therefore, this policy must be strictly enforced. Please do not ask your teacher or the offi ce to make exceptions to this policy.

Scholarships

To the extent that funds permit, CCSM offers fi nancial assistance to qualifying students in the form of reduced tuition. Financial assistance and scholarships are determined by using a sliding scale that considers family fi nancial resources. A copy of your most current Federal Income tax return and all W-2 forms must be submitted with each application. Reapplication is required for each semester (Summer/Fall). Call the offi ce at (909) 624- 3012 or visit our website for an application form. Scholarship forms for Fall 2008 are due August 22, 2008.

Liability

Upon the receipt of tuition payment, parents or legal guardians agree that CCSM shall in no manner be liable for any accident, injury, loss, theft, or damage which may be sustained on the premises in conjunction with any school-related activity or function.

Marketing

It is the practice of CCSM to take photographs at various events and use them to market future programs (may be posted on our website). If you would prefer that we NOT use your photograph, please submit your name and a photo for cross reference. We will keep this on fi le for 1 year and make every effort to avoid using your likeness.
